Where It Matters Most

The high-tech gold standard for this crisis — inhaled nitric oxide, and machines that oxygenate the blood outside the body — is expensive, demands specialized expertise, and is simply not available across much of the world. Affordable oral sildenafil has stepped into that gap. In small trials run in resource-limited hospitals, it improved oxygenation and was linked to lower mortality in newborns when the high-tech options weren't on hand. It's a quietly heroic second career for a molecule better known for something else entirely.

It's essential to be clear-eyed here. This is intensive hospital care, carefully dosed and monitored by neonatologists for a critically ill infant — sildenafil can lower blood pressure, so it's no casual intervention. The evidence, while genuinely encouraging, still rests on small studies that researchers agree need to be larger. And it has nothing whatsoever to do with the tablet an adult might be handed at a pharmacy counter.

What's left is a humbling reminder of how far a single molecule can travel — from a punchline to the hush of a neonatal ward — doing, in both places, the same elemental thing: relaxing blood vessels so that life can flow a little more easily.
